An ASP.NET, MS-SQL and AJAX web application project

An ASP.NET ,MS-SQL and AJAX web application project We are requiring the design of a simple ASP.NET 3.5 application. Time Frame : 2 to 3 weeks maximum The application will have authenticated users logging online who will: 1. Use 1 or 2 online forms (AJax and Web 2.0 skinned) to input data (stored in a MS-SQL 2008 database) 2. The authenticated users logged in can view reports of the data kept in the database. 3. The system designed to use skins and themes to provide a Web 2.0 feel. 4. There will only be about 4 or 5 screens in total for this very small system. 5. The user will also be able to print out reports against the data held in the system. All work performed will be assigned to us and copyright will be retained by us. You will have to sign a Non-Disclosure Agreement stating that you will not share information with anyone nor can you mention this project on any web page or website. This simple initial system will have further variations later and there will need more customization. As such we are looking for some one who can do this well, keeps cost down, and would consider a long-term relationship as we intend to develop more and more custom versions. Payments are through the GAF Escrow system. Payment will be made in 3 phases and only upon delivery of each phase will all code and data supplied first. There will be two other phases for this project but only upon successful completion of this project phase. A portfolio of any previous work or examples would help us select the best bidder. To ensure that we select the right person, we will ask the final chosen bidders to complete a simple assignment of taking a simple screen-shot and some data fields, understanding the structure and creating a sample MS-SQL database for testing. Please note that this part of the project is only Phase 1 of the 3-part project. There will be two other phases for which we will pay another two separate payments on the conclusion of each phase.
