Changes to Online Shipping Calculator (.Net) and updation of Shipping Rates

I have an online shipping rates calculator, that needs some changes to be done.
1) on my browser, the last column looks weird, and the table doesn't fit to the whole screen. Note the last column and the last row of the screenshot attached
2) move the Insurance column to the last column
3) squeeze the insurance column to make is smaller, it's the least important column, but it gets the most attention. make it small and on mouseover, the complete decription should be shown
4) squeeze the "code" column
5) remove the "HKD" rate column
6) Update the shipping rates based on the new data file that I provide
I will provide the link to see production data and also the source code of the calculator. Hosting details will also be priovided when we are ready to deploy the solution.
Need this done today.
This calculator also needs to be made more professional looking. But that is not part of this project. We may discuss it later so any suggestions to improve it further are welcome.
