Introduction page: This app is designed to provide you with a personalized list of vaccines recommended by the Centers for Disease Control and a diary in which to record the vaccines you receive. After answering several questions about your lifestyle and medical conditions a list of recommended vaccines is provided. When you receive a vaccination there is a page on which to record the date of injection. This will generate a calendar reminder for the date of the next injection. Please note these recommendations are for adults (18 years and up). Let’s get started. (There should be a button for “Do not show this page next time.”) 2. On this page there should be buttons for saved results, new, enter vaccination data,, Summary page (listing all their vaccinations and dates), If saved results pressed then go to Summary page. If new then go to demographic page, if enter vaccination then go to vaccination page. 3. Demographic page with first name, last name, sex, year of birth, medication allergies, pharmacy. 4. On the next page consult CDC website: [login to view URL] for quiz. Have sex and birthdate migrate from the first page. 5 When they go to next page there should be a list of recommendations for vaccines, this would be based on the quiz results. Button to save results under the users name. At bottom of page: Discuss these results with your healthcare provider 6 When they are given a vaccine there should be a page for entering date, vaccine type, serial number, “Who gave you shot” and notes. So dropdown to pick vaccine, or go to #3, click on vaccine which then takes you to page to enter date. 7 Based on vaccine the date for next vaccination should go into calendar and reminder pops up on the date of next vaccine. So flu shot yearly, but tetanus would be 10 years in future. If pneumonia vaccine (PCV13) then calendar reminder 6 months later to get PPSV23 vaccine. If HPV then 2nd dose reminder 1 month later, 3rd dose reminder is 6 months after first dose. 8 Page titled “Check Vaccine Availability” This is to list various pharmacy sites. Use the pharmacy name given in demographic page to put in pharmacy (hopefully ad by Google).. May use location in phone to find nearest pharmacy.
