Book Editing and Print Ready report and advice - experience in

I require a UK style book editor to check my book for the following. The correction of errors in spelling, punctuation, grammar and syntax. The correction of errors in sentence structure and language usage. The correction of factual errors where the information would reasonably be considered common knowledge. The highlighting of duplicate words where they appear close together. The return of manuscript with annotated corrections, indicating what has been changed, with an explanation where necessary. A full reading of manuscript. A detailed report outlining the strengths and weaknesses of plot, and pacing. An evaluation of writing style, which highlights issues or recurring errors. Suggestions on how any significant issues identified might be resolved. You would need to supply book examples, and quantify why your bid is as such and supply example evidence of publishing or self publishing knowledge. The book is attached and is 20k words so a very short How-to Book. I need to ensure that if I publish this to Create Space that their are no errors in this book - I have edited this three times myself and by one other party. I would require a sample piece from the enclosed draught and your price for: 1. Delivering a print ready copy for my uploading to Amazon Create Space and a number of others. In the correct format and layout. 2. Including all direction and the help I need to create a contents page, copyright pages, opening pages and so on. This helps the book look more professional. I intend to publish the book ready for POD and as an EBOOK, so some advice on how to convert this into EPUB, Mobi etc etc will be useful. It may be that the websites where you self publish might offer that service. I may need some guiadance This is a How - to book with a very small audience and so sadly I don't think i will sell more than 1000 copies so please bear this in mind :)
