create a page on our php website where we can have a quiz setup

we must be able to add multiple quizzes on the left of the page will be a category and subcategory (with the tests) to take. the tests will be multiple choice about 15-20 each per page. I can add all the test pages I just need you to do one. The test when we click submit must tell the how many questions were answered right out of how many questions giving them the % they scored. in addition when test is done and we see this must also show correct in green. incorrect in red. i have about 40 tests i want to add.
