Customize a MS Word Document

Need to reformat the attached word document so we can use it as a standardized form for quality control inspections. It is a report from another company that we want to modify for our own company so we want to remove the other company details and replace with our own. It will be re-used by our staff and they will fill in information. We converted it from PDF to Word format, so there are some formatting errors and problems that prevent us from doing things like adding photos or typing new inforatmion into some of the cells. We want someone to recreate it in MS Word Format 2010 format or newer and also edit with the following changes: 1. Ensure we can add our own information into each cell. This is a standard form we will use but need to add different details/images each time we use it. 2. Remove other company logo information(“ Asia Inspectaion” etc) and other company details from the header/footer. 3. Add our logo in the opening page. Logo attached. 4. Ensure we can input details into the fields. Right now in some sections, there are some problems adding our own text. 5. Ensure we can add/remove photos to the fields. Right now we try and add an image it does not work there are some errors when we try and paste images or embed them from our hard drive.
