Build a Website using PHP/HTML/CSS/MYSQL

Here are my project requirements: 1. Speak english, the software you are writing will be in english. 2. Comment your code! It also needs to be clean code. 3. No using someone elses framework, I want this coded in plain PHP I have already started the code process so please follow that, if you need help understanding it I will gladly teach you. 4. DO NOT BID IF YOU CANNOT START TODAY, DO NOT BID IF YOU DO NOT KNOW HOW TO DO THIS PROJECT. 5. Pay attention to my feedback ratings, I am a 5 star user. When I say you will get paid, it means you will get paid. I don't always prepay milestones because this is now the 5th time I have posted this project and the freelancer hasn't done what I have told him. I am now including a document to help please review the document before bidding. 6. I have a small budget for this right now, this project shouldn't take long. We can discuss this project in more detail, the document seems long only because I listed out the categories which I can put into the database myself if need be. My budget is no more then $60 for this project. 7. I have a deadline with this project, so I expect you to be able to work and meet that deadline. Thank you for your time.
