Build a Website which contains some dynamic components

I would like to build a website which has the layout similar to: [login to view URL], but it is a web portal for physician-coordinator-patients communication - And it should be viewed by major browers and iPad/iPhones and other mobile devices - This website should be able to switch languges between English and another choice or two. - Registraiton only 2 steps: email and password (super easy) - For registered patients members, allow the following functions: 1) login and fill basic information (one tab) 2) Provide basic medical information (one tab) and medical issue 3) Provide ways to upload supporting documents (one tab) - For coordinators 1) review and organize documents submitted by patients 2) can message patient members for clarifying questions and patients can message back - For doctors 1) review patients information and documents 2) reviewed edited information from the coordinators 3) ask calrifying questions to coordinators via messaging and coordinators can message back 4) provide a report per patients information - Allow payment from patients to doctor - Allow ratings of patients for doctor, and allow send "thank you notes" to doctors - Keep the time stamp, whenever the thank-you note is submitted, demonstrate one entry in the main page where everyone can see one transaction is complete. Use scroll to dynamically update the menu. In each entry demonstrates who is the doctor, who is the patient and the main description of medical issue - Please design the website logo
