Database and Website Needed

I need someone to build a website from beginning to end. The hosting account is already purchased and has PHP and MYSQL installed on the server. I will provide all necessary login information. This includes building: - the database - the front-end - some graphics The Database: There will be around 300 fields on the front end to put into the database. Mostly True/False, but some text and dates. The Front-end: There will only be two pages. The first page will be a questionnaire allowing the user to fill in all text fields and choose checkmark and radio buttons, and then a submit button. The second page will be a thank you page with a few image placeholders, and a download button. The user should be able to download all of the answers they chose from the questionnaire on the first page with some formatting on the PDF for presentation. Some Graphics: I can provide most necessary graphics, but if you are able to offer suggestions on graphics, then I am open to them.
