Convert PSD to AI with pantone colors

Hello, I have a Photoshop file that needs to be converted to an Illustrator file. The illustrator file must maintain similar layer structure so that changes can be made after the fact if needed (so basically just organize the layers and don't flatten the image). The colors need to be converted to the closest matching Pantone colors. In the final output, all of the blues should match in color and all of the reds should match as well, even if they differ slightly in the PSD file. I will be printing the final output on carbonless copy paper. I have attached a JPEG of the PSD file. Please let me know if you have any questions. Michael
