Add subscribe popup and buttons on webpage and scheduled emails

Want to add both a popup box that allows the user to add email and name when a button is pushed and some other input boxes in other parts of the web site that does the same thing.
The subscribers information will then go into a database and automatic emails will be sent to them customised with their name.
Once subscriber information is put in, it goes to another page to explain that they need to confirm email address.
user goes to email account and clicks link in email to confirm email which goes to another page of website.
Use [url removed, login to view] subscribing techique as an example.
