Beatiful Social Share Popup right after embedded youtube video end.

Hi! I am looking for a web programmer who help finish my idea and my project. I posted youtube videos via embed frame. I want a popup (or could be better in overlay the video player than 9gagtv - check on the site the video end) which give users social share possible. "You liked this video? Share on Facebook (button) | Share on Twitter (button) Replay (video replay button)" dark grey backgound, white text, branded facebook/twitter button - MINIMAL style SAMPLE ATTACHED! What i have now? I have a javascript which detect when youtube video end and can appear a popup with alert. This is not good for me because i can't add html/css and create my share possibility. Need replace the "alert" part to a code which appear an attached box. The project half is completed because my javascript. I know the php url for buttons.
