Code Needed to Dynamically Create Form Inputs

Need code created to dynamically create form input fields on a web page. Inputs need to be in a vertical column and when button is selected to create additional inputs they must be created in a column next to the original column. Also a remove button is required to remove the last column of dynamically created inputs. There will be seven input fields with on being a select box. Inputs will consist of fields named: Garment Type (text input) Garment Color (text Input) Number of Front Colors (text Input) Number of Back Colors (text Input) Type of Service (Select input) Estimated QTY (Text Input) Due Date (text Input)
