Design Logo and Two Application Screens

I'm building a media management application and need some help in designing two screens in the app and a logo. 1) Logo The name of the App will be "LokaVid". "Vid" refers to "video". 2) Add Screen ("[login to view URL]") This is the page where users will go to add their media. They will have a choice of either adding "Movies" or "TVShows". When one of the buttons are clicked, it should change colour or in some way indicate that it has been selected. When the user selects folder, some text should appear (maybe just above the "OK" button?) that states the filepath they have selected: Eg: Folder: C:\Users\User123\Desktop\somefolder 3) "[login to view URL]" is the home page. If "Movies" is clicked, it will take the user to a page containing previously added movies. "TVShows" will take them to a similar page but for TVShows. "Add" will take them to "[login to view URL]". "[login to view URL]" is what has already been designed for this app. I've included this so that your design can have a look that maintains the overall feel of the app. The end product should be 2 photoshop files containing the designs for the two above mentioned screens and the logo.
