Create MS Access 2007 Database to track client appointments

I need an MS Access (2007) database to track my client appointments. I need to include the following:
* Client information:
* First name
* Surname
* Address
* Suburb
* Postcode
* Home phone
* Work phone
* Mobile phone
* Email
* Health fund (drop-down list, which I will provide you entries for)
* Date of birth
* Comments (large field)
* A form to be able to add new clients with the above information.
* A form to be able to add new appointment details for each client. I envisage this to be the client form at the top, and underneath a table with the following information:
* Date of appointment
* Duration (mins) (drop-down list, which I will provide you entries for)
* Cost ($)
I want this table to show all past appointments for the client who's details are open, with the ability to add new rows for each appointment.
* A reporting feature where I can view the following reports:
* A report for a particular client, where I can see the history of their appointments and the amount spend.
* A report where I can specify dates to view all client appointments in a particular range, and the amount spent.
* A report where I can see how many of a particular duration have been entered.
* And I need the whole database to be user-friendly and smart-looking :-)
