Defect Tracking System

Hi All,

I need a custom defect tracking system. The specification is attached. Please bid only if you are confident to deliver the project in 5 days time. There are only 2 pages for this application. I want to see a prototype before selecting the winning coder/company. Also he/she should be available on GTalk to discuss this in detail.

This is a very very urgent project and if the person delivers the project successfully is likely to get more development work.
