ASP Order Form w/ Verification & File upload

I need a coder to take 2 order forms, write it to an Access database and send two emails. The form will require field verification (javescript). The user will have the option to upload an image file. This is for an ordering system that will behind an SSL. My Windows Server requires that we use ASPMAIL for the email, and for the upload mysmartupload. PELASE ONLY BID IF YOUR EXPERIENCED with these two compenents. Step 1. The user will fill out a form with approxiamtely 12 fields. Javascript will verify that required fields are filled in. Note - 2 different fields on the form will become required if they select yes from a radio button. They have the option to uupload a file. This file must be given a unqiue name so if they call it [login to view URL], the next users "[login to view URL]" does not overwrite theirs. STEP 2. A Verification screen, asking them to review thier fields. At this point they could either click to EDIT which would take them back to a populated form, or they could click submit. STEP 3. The checkout. They jsut fill out another form with name, address, credit card number ( must be 15 or 16 digits), and an expiration date, and a ship to information. STEP 4. A Verification appears with a price, (Only 2 options, from the size they selected in step 1 above, plus a flat shipping amount of $ 5.99. STEP 5. The database is written to, and an email is sent to us, and an email is sent to the customer, with the details of the order. PLEASE NOTE - Only one record in the database per order please. Please note all the HTML is done. In fact the ASP is actually done, but does not work, and writes the order to 3 different tables. So I want to scrapp all the ASP and start over. I need this back as quickly as possible, and please note, that the component requirements mentioned above.
