Create Office Locator ASP

I need a Office locator search done. The office db can be in DBF using Apollo or MS SQL If you check <[login to view URL]> this url I control so for the moderator it will not change and my documentation shows screen shots of this page and how it works. I am upgrading their site to a new look and need this code for the office locator rewritten. I want it to be template driven. Header html file location html file footer location file. the new site design is [login to view URL] so you can try and match this in the original design I want the ability to search by City or City and State or Zip - if zip is entered all others should be ignored or state or office name I will provide the database in a excel file so you can test. You give me the sql command to make your table and import the table and I will do that on mssql server Final product will be a Form command that does the search and show the resulting output like the current site is now. But now we will use templates so designer can modify templates to make proper output without changing code. Form command must also log every search to a database giving date and time along with criteria inputed Must use indexes to make as fast as possible. You will need to use your own mssql to develop for mine can only be accessed by my server.
