Convert PDF Job Application to HTML Web Form

I have a 4 page Job Application that needs to be converted to an HTML web form. Upon submission, the completed form will be 1) displayed for the user to print, 2) emailed as a PDF attachment, and 3) the fields will be stored in a row of a CSV file on the server. The form is 4 pages total and contains approximately 90 fields.
Need project completed within the next few days or I'll have to do it myself :)
