Custom PHP Donation Form

I need a custom PHP donation form, similar to the one at the following link: [login to view URL] Key functionality: - Input details similar to those in the linked form above. - Pass that information to a review page to display the inputted information. - On submission of the review page, redirect to Paypal to complete donation. - With donation complete, redirect back to a thank you page. - At some point, email the information from the initial input form to a specific email address for internal records. Preferable this would happen after the donation is complete through paypal. I already have a website set up and can create new template pages for the input form, review page, and thank you page. I basically just need the required PHP to insert into the main content div of the page.
