Alter Ajax dropdown

We have an ajax dropdown form that we would like to make 5 design and functionality alterations to (without increasing the query time or load):
1) If the selection does not match the main name, but the alternative name, only show the alternative.
2) Make the matching text bold.
3) Change the design slightly.
4) Do not show the "region" unless there is a duplicated location within that same country
5) If the user types in a comma, directly match a country or region after that comma, including any spaces (anything after comma - eg. Santa fe, north carolina or San Francisco, United States).
