Custom HTML Links Page for Tutorials/Demos

We want an application created for us that will be web-based (php/mysql), and we will use it to automatically generate a [login to view URL] page for our customers complete with hyperlinks to demo pages. There are two parts to this job. An admin part that allows me to build a database of demos and links, and a html page building part that will automatically build a working html page based on the information entered (header/footer/filename/links selected). A complete description of the application is included in the attached text file. IMPORTANT - PLEASE, no blanket bids will be accepted.
