Custom component and module for Joomla timesheet

I am going to need a couple things built for a joomla site. We are using Project Fork - and want a time tracking solution built. We have a very basic need, a table for employeeID, JobID, Date, Hours It will need a clean form that lets a user choose a project from a drop-down (pulled from the project fork list) and then a date picker, and a text box for hours worked and breaks taken. Then on the backend there needs to be a page that can dump this in a quickbooks time sheet format.
