Design an Email Opt-in Box For a Wordpress Blog

I have created an email newsletter called using Aweber. I would like to embed an email opt-in box into my wordpress blog above the fold. I would like an email opt-in box similar to the one on Entrepreneur's Journey ( [login to view URL] ) and I have already created the video that I wish to embed into the opt-in box. I require assistance with: a) Creating the box b) Creating the book graphic that best represents the newsletter c) Embedding this opt-in box into my blog above the fold in a manner in which it blends seamlessly into the blog and doesn't interfere with anything else. ## Deliverables I have created an email newsletter called using Aweber called "**The Immigrant Survivor Guide Newsletter**". The text should be as below: * Someone will have to provide their first name and their email to opt-oin. * It shall have the words, "Free Instant Access" on the button one clicks to opt-in.
