Breezing Form Project

I have a form that we want to build. I would like it to be built in Breezing Forms. It has certain qualities that are outlined here. We need someone that knows how to build a form that will calculate several fields and display a page (depending on their) score. We have built a [login to view URL] that will walk you through each of the pages and what we are looking for. The deliverables on the form are: Deliverables: Our understanding is that all the questions which are currently present on each tab are fixed. We will provide an option to review all the inputs provided by the user on the review page. Ability to edit the answers by clicking on the "Change Answers" button. Please note that user need to traverse from the beginning to edit the answers and it is not possible to redirect them to a particular section. After reviewing the answers, user will be prompted to enter their name and email address. On clicking on the Submit page on the review page, we will redirect the user to a page where the calculation image/animation will be displayed. Test report will be emailed to the user email address and to admin email address. User will be redirected to SLF score page to view the score. Based on the score a pop-up window will be displayed to the user. Our understanding is that the pop-up contains an image and static content. You will provide us with the details of the content and image that needs to be placed in each pop-up. All graphics and calculations will be provided. Please provide me a bid on this. Eric Kleinsorge
