Custom payment method for Opencart 1.5.6

We are in need of a custom payment method.
Really quite simple - when a customer chooses this method, he/she can add personal information (3 text fields and one dropdown menu) right under the payment method. No gateways needed, pretty much like a copy of bank transfer module but with extra text fields that show up in admin.
