Bid Sheet Entry Forms

I need a bid sheet for a construction company put together that will have the company logo and information at the top, with a field at the top where the customer's name and info can be typed in. There will be an area for each kind of work, (excavation, concrete, masonry, etc) and there needs to be a price field for each item in each section where the price can be entered, and a total at the bottom that calculates the total cost of the section as the prices are entered. Then, there needs to be a total for the form at the end that calculates the entire form's total. Now the question is what program or method should be used to accomplish this. I want the client to be able to use the form preferably without having any additional software. My first thought is to make it a PDF. An excel form would also be a possibility. The third possibility would be to make it a web form where that prints out or can be emailed, but the print out would need for any included images to be high resolution. I've attached an extremely rough version of two forms they are currently using by hand. (excuse the quality) - these will pretty much be all the exact fields used on the new form. The two forms will be used together as one. I will provide text / info, logo and possible layout for the form to be added to, depending on the route that is taken (pdf, excel, web form, etc). This form needs to look real nice, clean look overall. All questions and comments are welcome. Buyer will receive exclusive and complete copyrights to all work purchased.
