Need to convert excel into reliable access database
**Quality program-**An employee is monitored twice a week on the items listed attached
**Current Quality spreadsheet**(A-Q are usually hidden)
**Employee table**- Consist of employee name, department and the team (Team Lead) their on

**Point Value**-consist of criteria and their point value
**Table of reviewers** - who monitored the employee- drop down
**Instructions
Need**
When a reviewer opens DB, opens to forms that gives option to monitor or reports
When click Monitor
Opens a form similar to the spreadsheet but for one instance (the spreadsheet shows five instances-twice a week)
The reviewer will enter the following:
* Their name-reviewer (drop down)
* Employee they are going to monitored (drop down)
* Caller's Name:
* Policy/Contract #
* The date and time of the monitor (monitoring is not always real time)
* Category and criteria (like on the Spreadsheet) with yes or no option boxes
* Like the spreadsheet - the Security Check should be first (very important item)
* All criteria are yes/no except Provide Accurate Information (point scale)
* Under the criteria should be the 1st Call Resolution section (Check if Resolved)
* Scoring section should be locked. can be in a list box or whatever is best and looks good

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
