Data Collection Script

We are in need of a small script, preferrably based on Visual Basic, or a web interface, used to collect computer asset information.? ? The objective? is to have a Form pop-up on the users screen, or have them go to a link where they can fill out a Form requesting their PC information.? We basically need to have end-users register their Computer information.? In addition we will need them to provide their location information including address, floor, room, and name.? The PC information should include Barcode, Serial Number, Make, and Model.? Their should also be clear instructions available on the form describing these data items to the users.? The information collected should be stored in a database or table (e.g. Access).? If necessary, clear instructions? should be made available? to set-up the back-end? database or table. The solution provider should also recommend a delivery mechanism to the user, i.e. logon script or web link via email.? ? Preferrably, the provider should provide? an operating procedure from setting up the collection tool (front end delivery and back-end) to executing the whole process. Thank you? ? ? ? ? ?
