I need a employment application page designed for my Wordpress sight.
The application will have several sections, below is an example of 1 section "personal info"
Personal Information
Name in 3 separate fields (first middle and last)
3 address' broken into street address, city, state, and zip. (Current, Previous, Permanent)
Date of birth if under 21
2 phone numbers
and email address
all fields must be validated for content/format.
it must work with my current theme.
other sections will be, position and location based, general info, education, employment history, and references
The form must generate a PDF that is emailed to a "back end" configurable email address. (other back end options a plus)
This is a 3 page employment application on paper so please don't bid if you are not ready to complete the job.
