Create an online form with Zoho Creator. Some basic scripting will be required.

Make one change as follows to the "Home Phone" and "Cell Phone" fields.
In the U.S. phone numbers consist of ten numbers.
If a user sees a plain text box, they will input numbers in one long string (e.g. 5555551212).
However, I want three separate areas on the same line so that the number is broken up into three segments.
So in the above example, a user would instead input 555 555 1212 in three separate areas. (3 numbers in the first area; 3 numbers in the second area; and four numbers in the third area)
It would be good if the areas were separated by a hyphen.
An alternate way of handling it is for when the results of a returned form are emailed to the account holder (me), to have the string of numbers separated into the more readable fashion of 555-555-1212. So the single text line can remain as long as the line is broken up in the email that is generated to me.
The goal is just to have the numbers more readable to me when I get emailed results of completed forms.
You will create a new account on Zoho and build the form inside that account. Upon completion of the project, you will show me the completed form and turn over the password to the Zoho account to me, after which, I will change the email associated with the account.
