Create Interactive PDF Forms

We need an interactive PDF form created, where it can be typed by computer, then printed and saved. We also need the ability for blank template copies to be printed and filled out by hand. We estimate the total project to be 8-10 pages in length. We currently have a multiple existing forms that we can use as a guide. We plan to take the best features from each form, add some new features or fields, and create this new custom form. We will email this data to you upon the start of the project. It must be professional in nature and include the use of our existing corporate logo, etc. This form is a financial survey type form, so there will be many text fields, and also many number fields. Some of the number fields will need to perform basic math calculations. Turnaround time should be less than two weeks, start to finish. We would anticipate several rounds of revisions until we have the final product. After the final product is received, we may need you to edit the forms from time to time upon our request, but that will be paid for seperately. We will pay for this project by the project, upon its completion. We would like to see an example or two of your previous work similar to this project. There could be other work needed for similar projects in the future, if you do a good job. All information about this project is confidential, and may not be shared with anyone. Thank you.
