I just finished 2 courses of HTML5 and CSS3. I can deliver hard questions (though there aren't that much really). Just so there is no misunderstanding: You want 40 questions, coded, on HTML5, delivered in plain txt, right?! Because I don't understand php, nor will I be able to make a quiz for/in php..
