Create a PDF Form to be Populated with XML Data

I am looking to have a program built that will take XML data submissions from my online rental application and create an easy to read PDF rental application that only shows the data that is submitted. So, basically if someone has a co-applicant then it should show the fields for a co-applicant and they should be populated with the data that was submitted by XML....but if they do not have a co-applicant then those fields should not appear on the application. I have attached a sample rental application that should give you an idea of how many form fields there are that we would need to have available if all sections applied to the submission.
