Custom Admin Input Page Into MYSQL & Retrieve through PHP

Hi there, we need a custom admin page developed, the end result will look something like the image attached.

The admin will be able to access the page and input two pieces of data against an autogenerated row number (e.g. 1,2,3,4) and save the data to mysql.

The data should then be retrievable and auto pulled into php.

Here is our PHP code which the input data needs to be pulled into, its a file called [url removed, login to view]:

The above code contains 4 entries of data. The variables

$titlekeywords

and

$products

are the two pieces of information that the admin enters into a row in the admin page.

These php queries should continue to expand as the admin adds more data into the admin page. There may be a better way of doing this, but essentially the admin needs to have a user friendly page to enter the information into.
