Checking-in software for trucking company

A trucking company needs software that they can use to check in there trucks using a form on a tablet instead of the current paper based method they currently use. It would have a place for the truck number, date, time, all the items on the truck that need to be checked eg: windows, tyres etc. and space for a comment at the bottom. It will have to be uploaded to a database and must be able to be recalled into either a spreadsheet or something similar for printing (if need be).
