Design multi language form

We have a number of forms for tax authorities in Spanish. These documents consist of a fixed context - which has to be translated in English, French etc. - this translation is provided by us. There is the variable part where the name of the company, address and figures are posted. What we need is - with restricted access, either on a web or on a word doc (with watermark) so that someone can fill in in Spanish the parts that are variable and the data appear in the English, French etc. form. Ideally this would be on a site with a time restricted possibility to attend, and also that the european form for numbers 1.000,00 is changed automatically to the English form 1,000.00 I attach you a sample form so you can see what it is about.
