Multi step form on gsrthemes template

Develop a 3-step PHP form similar to [login to view URL] , so with Ajax or Jquery without reloading the page. This form must be well documented and easy to modify. The differences are that 2nd & 4th from the example page must be merged in 1 step. It must look like new modern flat designs the form must look something like this: [login to view URL] For the uploading of the pictures this script must be used where people can upload multiple images at once. [login to view URL] Unlike the example page the pictures should be added to the form before sending the form out. These pictures must only be send by mail as attachement and not stored on our server. Only image file types should be allowed like: .jpg, .png, .gif, ..... The form must send out a confirmation e-mail to the sender in html format. The form must send a html e-mail to the administrator e-maill with all the result of the form choices, fields and images. The mail that is sent to the administrator will be saved in a small text file, just the data of the forms, not the pictures. (This is as a backup)
