I need some Graphic Design

I need a designer who has also excellent skills in Microsoft Excel. I have a questionnaire attached which I need to structure so that it is easily read and completed by assessors and in which certain fields are linked to the front page which is turn creates an executive summary looking like infographics such as pie charts, bar charts etc. So for example I would see an extra field at the end of category where it asks for the score for cleanliness, food quality, etc and this is fed into the front page chart creating a bar for each category and then another for the total. Also I’d like a panel (infographic style) showing key positives and another for key negatives. This could be driven / populated by specific comment fields. Please can you submit similar work to demonstrate your abilities in this area. The budget is €250 for a template I can use, adapt, add and delete questions without affecting the front page executive summary. Thank you. Regards, Alan Keane
