Write a form-filling script (and simple browser extension to attach it)

I need a script to be written (using JavaScript and jQuery), in the form of a simple Firefox browser extension (using WebExtensions), to fill in the form fields on this page: [login to view URL]
The script will eventually be integrated into a larger Firefox extension. It will send a message at the start to request the data, which will provide it with the information it needs to fill in the form. An example set of information is below:

Each form field has to be filled with the relevant piece of data stored in `data`. For example, the form field asking for your number plate will be filled in with `data['number-plate']`.
A list of all the variables in `data` will be given to you if you accept the job, though I will not be able to send it to you until Wednesday, so you won't be able to start until then. The variable names are self-explanatory so you should be able to work out what fields they correspond to.
When writing the code, please work your way down the webpage in order the fields appear. The variables are in no particular order.
Next to each variable states the data type. Some variables will be arrays, because multiple options can be selected.
Some variables there will be a limited number of values (for example, licence type), those possible answers are listed indented underneath the variable.
The code must also be well-commented and indented (see the attached image, your code should look like that).
An example extension will be provided just to show you the messages you need to send (using [login to view URL]) to get data and attach the form again (in case of a page reload). You may also use that [login to view URL] for your extension as it already has the code to respond to those messages.
