I am looking for a developer that can build me a GUI application using Java that allows the user to ADD, UPDATE, and DELETE data from a Microsoft SQL 2014 Server database. This is a simple database with 6 tables. Very few attributes in each table. I would like to keep this project really simple and basic. This is for a school project, so nothing fancy. My budget is negotiable, but I would like to keep costs low if possible. <$100 would be ideal, but I do not mind paying more for extra work.

To give you an idea of what this database is about. A friend of mine runs a insurance company which sells home, auto, and health insurance. I am creating a database and GUI application for him so that he can CREATE, READ, UPDATE, and DELETE employee and client information. This database is very basic.

I have included a sample ERD, CREATE table scripts, and pictures to give you an idea of what is required. If you could also add a QUERY tab, where I can run custom mySQL queries and return results that would be great. Additional information can be provided if needed.

There are just a few requirements that I am looking for.

I need this to be completed by December 1, 2016.

The GUI must be run through a JAR file to test GUI functionalities.

A readme file or simple video on youtube explaining how to use the GUI.

Be available to make changes if necessary after completion of project. (I do not mind pay a little bit extra for this.
