Deploy Rails application on production web server

We need help to (re)deploy a small ready-to-go Ruby on Rails stand-alone web application. The application was coded for us around 2012 and that ran without any issues to around 2014 (then taken down due to hosting db and storage limitations).
We want a fixed quote for the deployment job, and would prefer someone who also can do follow up/support for us on this application at a per hour rate.
Suggested fixed app deployment quote: 400 USD
Suggested support rate post-deployment: 40 USD/hr
# Deadline: Application must go live before December 5th, 2016.
# Tasks:
1. Consider if application can run on our managed dedicated linux server now hosting our php/mysql-websites. Ruby on rails is now installed on this server, and further server side extensions may be added by our supplier. Our supplier has limited experience running Rails applications on their servers.
2. If it is not feasible to run the web application on our own server, find a reasonable priced and good hosting service.
3. Deploy web application.
4. Make sure the application run fine with stable and secure version of Ruby on Rails: If needed, adapt script.
5. Change a default checked input check box to default unchecked in the application.
# About the application:
The app let visitors generate a “green” gift card, giving away environmental friendly gifts like for example inviting to dinner, going to cinema etc. The visitor may choose among a gallery of readymade cards or upload their own photo to make their own personal custom gift card. The card can be forwarded by e-mail directly or printed. The user may choose to make his/her gift card publically viewable or keep it private.
