Add a Mailchimp Signup Form Sticky Popup Widget to my Self-Hosted Signup Form

Nothing huge, I just need a Mailchimp signup form that looks like the popup ones on the bottom right-hand sides of these two sites (but for Mailchimp): [login to view URL] and [login to view URL] I like the orange asterisk and the social media icons on the second link.
Feedburner has a similar sticky popup plugin here [login to view URL] that works and looks almost the same, and it looks like it has mailchimp options, but I can't get it to swap out the feedburner code for the mailchimp code in the actual widget (the code I'm referring to is in the [login to view URL] file in the Feedburner Follow Me widget that I have installed on the site). If you can get that one to work, great. If not, whatever alternative that works the same is fine.
I like the verbiage and the actual subscriber counts in both the examples in the first paragraph. The site to install this to is here: www.lenastark.com. If the work on this goes well, I have a few other very small theme customizations as well that I will be putting up in a separate job in the next couple months. Thanks!
