Background Info:
Add HTML form to PDF function to existing website. The website was written in classic ASP. Hosted on Windows 2008 Server, shared hosting, server supports .asp as well as .php.
Notes:
Prefer Classic ASP, MS SQL Server, PDF, JavaScript, PHP is also considered.
Code should be clean and well commented for future modification.
Populate PDF template using .fdf or .xfdf.
Not interested in installing server components such as aspPDF.
Templates (see attached) should not be modified, except the text boxes name if necessary.
Phase 1:
Create html form (input page) based on application form V2011A
1) No login required
2) 6 sections:

3) Form fields' name should be thoroughly considered in order to match SQL table and PDF template fields.
4) All sections need to be on one page. Section tabs on top of page. Show one section at a time once the tab clicked, hide other section. Show Form instruction section by default.

Phase 2:
1) Create table in MS SQL Server (version 2005)
2) Table fields' name and property should be thoroughly considered in order to match form fields as well the PDF template fields.
3) Link html to SQL Server.
4) Once user click "Finish"

[login to view URL] Confirmation Page with user name, visa type, service level, shipping address, Application #. Remind user to print page for future reference.
5) Need script to create the table with.
Phase 3
1) Add form validation mechanism using JavaScript. Check data for completeness and correctness. Will provide details later (will attach file).
2) Add form help message on mouse over or click.
3) Embed a random code generator, user must type in the correct code in order to Finish and Save to SQL Server.
Phase 4
Create retrieve page and modify page allowing user to come back to retrieve and modify data.:
1) User should be able to retrieve data with Application #, year of birth, and last name.
2) Once data match, display modify page showing previous data, user can modify.
3) Once user click "Finish"

Phase 5
Add PDF Generate function to both Input Page and Modify Page.
Details:
1) Using the data from the form page or SQL Server to generate [Timestamp].fdf file or
[Timestamp].xfdf file
2) Using .fdf or .xfdf file to populate the V2011A PDF template on user screen. User can download, print or email the PDF. There is very good code sample in PHP at the link below:

Phase 6
Do the same for V2011B form.
Respond only if:
1) You are VERY comfortable with classic ASP, MS SQL Server
2) You are good at form validation using JavaScript
3) Understand the logic to populate PDF template with .fdf file.
4) You are available
5) Understand the nuances of developing client side applications.
Expected duration of development: 2 Weeks.
