Create a website to redeem reward points

We created a network program called "Nightclub VIP Reward Card". We will be selling this VIP card to our loyal party goers, who already comes to our events that we do in diferent nightclub. We are using a software called "PatronWorks" to give the points to the our members. They use a SQL database and give us the ability to export the data in .cvs format. I would like to create a website where these members can login and check their account balance, show them how many points they have accumulated, etc... Also i want them to be able to redeem those points for merchandise. We need a page where we can add the products we want them to buy with their accumulated points. And track how many we have available, some type of inventory. We need a shopping cart, but at the time of payment it won't be with real money or credit card, it will be only to be redeemable with points. I also will need a nice and simple backend to add/edit/delete products, add new member, import member from the .cvs file generate by the software Patronworks. Check orders and change status. I'm looking for somebody with good design skills and php/mysql knowledge. If you can follow this sample design which i did in photoshop will be great.
