Help with PHP and Zend Framework Application - repost

This is a re-broadcast of an earlier job. I had awarded the job to a freelancer that turned out to be a FRAUD. So I want you to know that if you are awarded the job, I will pay the milestones (a small one to start) and then the balance ONLY when you have proven you can do the job. You must also be capable of being on Skype with me and your English must be very good. I will want to dialogue with you before I award the job. I also want to have this completed quickly if possible. I have a problem with a program that was created for me (it is inserted on my website) The program takes information from the viewer who is looking to schedule an appointment. When the person "submits" that information, Google Calendar is communicated with and the program then displays several dates that are available. After a particular date is selected, Google Calendar is notified and the appointment is blocked out. Here's the problem! This application was created with Zend Framework 1.0. The company that was hosting my website no longer supports applications running Zend Framework 1.0 and now only supports the current version. Is there anyone that can convert my application to the current version or re-write the application in another format? You'll see the second page, but NOT the available dates because the application has ceased working. I have attached an overview of the way the pages were displayed and working.
