1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
## Deliverables
We are in need of a form processing script, that will accept information from an HTML form, and then store the informtation submitted into a MySQL Database. Here is the form: http://www . corporatemeetingsunlimited . com / [login to view URL] (Put the URL back together in your browser) Here's the schematic for the operational part of your PHP script: - The form will contain varying data types (i.e. text, numbers, special characters, etc.) - The information submitted will need to be processed by your PHP script. Your script should do the following: -- Store the info in a MySQL database. -- Send the user to a pre-defined page (i.e. [login to view URL]) -- Check the referrer to make sure that the form is being submitted only from an allowed list of URL's (we will define these URL's in the [login to view URL] of the script). If the referrer is not in the allowed list, the script should simply redirect the visitor to a pre-defined page without processing any data (we will define this page in the [login to view URL] file of your script). Now, the script will also need to have a back-end. The schematic for the back-end will need to be as follows: - The admin should be able to look through all submissions, in user-friendly manner, as the end-user of this script, will not be us, but will be one of our clients, who is not very computer savvy, so it will need to be user-friendly enough for her to browse through all the submissions. - She will need to have the ability to sort the data in any way she chooses (by name, location, or any other form field value that exists on her form) - She will also need the capability to conduct searches on the database by specifying certain criteria (i.e. Hotels with a Room Rate of $100 or less, for example.). Your script will then need to return the results, sorted in the order she indicates. - She also needs the capability to export the data into an Excel Spreadsheet (.CSV format). Your script should produce output similar to the Excel Spreadsheet we attached in the .ZIP file, as this is what she is looking for. - This script needs to be scalable so that we can use it with ANY type of form, not just the one in the above example. We should be able to control many aspects of the script from within the back-end or from within a config file ([login to view URL] or something similar). We are usually flexible on deadlines, but this client needs this script done by a certain date, and therefore, the deadline on this project is firm. This should be an easy, straight-forward project for an experienced PHP Coder anyway.
