Custom Checkout Process For Magento 2

We need a highly experienced Magento 2 developer to customize the checkout process on our Magento 2 site. Magento is already installed and configured on the server. We are only looking for someone to come in and work on the code. We are adding in a new checkout step with custom order attributes, and a couple image upload fields that needs to be saved to the order when it is submitted. The new checkout step, attributes and form handler all need to be created. I have detailed specs on each input field that we need to add (requirement, backend name, frontend label, type, etc.). I've also attached screenshots of what the checkout frontend needs to look like, to give you an idea what we need to accomplish. Billing address, payment, and shipping methods will also be removed, since they are not needed for our custom store. We need to have this project started right away, and finished in 5 days or less.
