Small program modification in C C++

We have small software program written in C.
It is downloadable off our website, I can send you the link.
It is very small. It scans the Windows Bios to identify the computer model and reports back the information on screen.
There are a few changes I need to make.
1.
I want to add two new buttons for customers to give feedback (see image). Yes the scan was correct, no the scan was incorrect. When the customer clicks the button we need the system information added to our database. This database is hosted on our server and is MySQl and PHP.
We will do the server side programming of the database and php. We need you to recommend and advise on the best way to do this. Security and privacy is important.
2.
There is a contact us page. This auto fills a form on our website with the customers model information. We need the model add from item 1. to happen if the customer clicks on the contact us page and visits the form. Prefer if the model information is added to the database when the customer clicks the button on the program not if the send the form on the website.
3.
Search our Store button. This function needs fine tuning. We have recently updated our search function on our website, and we may have broken its functionality. We will fix the code on the server side, however we need your help to make sure ht e program is communicating properly to the program.
We have all source code
The project is completed when we have source code returned, with changes, the way it was delivered. We will compile the final code and check it is working.
