Video Upload Function for WP XXX Site

For my adult website, [login to view URL], I need to create a simple custom upload page so viewers can upload their the own videos to my website. Once the file is uploaded to the media library in WP, it will create a pending post with user-selected tags, category, title, duration. I need admin view to approve or disapprove the posting of the video. I want to essentially copy the same look and functionality as this page. [login to view URL] (including the ability to drag file over the area to start download). Test that sample page with a video upload and you can see the next page of details I want copied as well. Not everything on that secondary page, but some. Also, I need to setup and integrate Profile Builder into the overall user flow: Register, then have access to the forum and also access to upload the video.
